Consumer Health Data Privacy Policy

Depending on the questions presented in our product recommendation quiz and the answers you choose to provide, we may collect the following categories of consumer health data.

We collect consumer health data from the following categories of sources:

• Directly from you when you voluntarily answer questions in our product recommendation quiz.
• From your quiz interactions, including selections, responses, and other actions taken while completing the quiz.
• Information generated from your answers, such as product recommendations, preference scores, quiz results, or other conclusions generated by the quiz logic.
• Our quiz technology provider, RevenueHunt Product Quiz Maker, which processes quiz interactions and related records on our behalf.

We do not purchase consumer health data from data brokers for use in our product recommendation quiz.

We collect and process consumer health data for the following purposes:

• To operate and provide our product recommendation quiz
• To understand the answers you provide and generate personalized RevitaPlus product recommendations
• To display and maintain quiz results or recommendation records
• To provide customer support and troubleshoot issues relating to the quiz
• To understand and improve the performance and functionality of the product recommendation experience
• To maintain records reasonably necessary to operate the service and respond to privacy requests
• To comply with applicable legal obligations

RevenueHunt Product Quiz Maker

Our product recommendation quiz is provided through RevenueHunt Product Quiz Maker, operated by Durian Capital Inc.

RevenueHunt processes quiz responses, selected choices, recommendation information, quiz identifiers, and related technical information on our behalf for purposes such as operating the quiz, generating product recommendations, providing quiz functionality, analytics, and technical support.

RevenueHunt and its authorized service providers may process information as necessary to provide and host the quiz service, subject to their contractual obligations and applicable law.

Shopify

Depending on how the quiz is configured and whether you provide contact information, RevenueHunt may synchronize certain information with our Shopify store, such as contact information, marketing consent status, customer identifiers, or a quiz response identifier.

We do not intentionally use raw health-related quiz answers in Shopify advertising audiences.

Third Parties & Affiliates

We do not currently share consumer health data with third parties or affiliates for their own independent marketing or advertising purposes.

We do not intentionally disclose health-related quiz answers to Meta, Google Analytics, Omnisend, or other advertising or marketing platforms for the purpose of creating advertising audiences.

We may disclose consumer health data where required or authorized by applicable law or where necessary to comply with a valid legal process.

Categories of Consumer Health Data Disclosed

Consumer health data processed by our quiz provider may include quiz responses, selected choices, recommendation results, preference information, quiz identifiers, contact information when voluntarily provided, and related technical information necessary to operate the quiz.

Sale of Consumer Health Data

RevitaPlus does not sell consumer health data.

If our practices were ever to change in a way that would constitute a sale of consumer health data under applicable law, we would obtain any separate written authorization required by law before such a sale.

Cross-Site Collection

We do not knowingly authorize third parties to collect your consumer health data through our product recommendation quiz over time and across unaffiliated websites or online services for their own independent purposes.

Where required by applicable law, we obtain affirmative consent before collecting consumer health data for a specified purpose.

Where separate consent is required before sharing consumer health data, we will request that consent separately from consent to collect the information.

We will not intentionally collect, use, or share additional categories of consumer health data, or use consumer health data for materially different purposes not disclosed in this Policy, without providing the disclosure and obtaining any affirmative consent required by applicable law.

You may withdraw applicable consent for future collection or sharing by submitting a Consumer Health Data Request as described below.

Depending on where you live and applicable law, you may have rights regarding consumer health data associated with you.

These rights may include the right to:

• Confirm whether we are collecting, sharing, or selling consumer health data concerning you
• Access consumer health data associated with you
• Request information about recipients with whom applicable consumer health data has been shared or sold
• Review consumer health data associated with you and request correction of inaccurate information where applicable
• Withdraw consent for certain future collection or sharing of consumer health data
• Request deletion of consumer health data associated with you
• Appeal certain decisions if we refuse to take action on a valid consumer health data request
• Exercise applicable consumer health data rights without unlawful discrimination

How to Submit a Consumer Health Data Request

Please use the subject line “Consumer Health Data Request” and provide enough information for us to reasonably verify your identity and locate the relevant quiz response or other records.

You do not need to create a new RevitaPlus account in order to submit a request. If you already have an account, we may use account information as part of the verification process.

If we cannot reasonably authenticate your request, we may ask for additional information that is reasonably necessary to verify your identity and locate the relevant data.

Response Time

We will respond to authenticated requests within the time required by applicable law. For laws that provide a 45-day response period, we will generally respond within 45 days and may extend that period where legally permitted if additional time is reasonably necessary.

Deletion Requests

When applicable law requires deletion, we will take steps to delete consumer health data covered by the verified request from records under our control and will notify applicable processors, contractors, affiliates, or other recipients when required by law.

This may include notifying RevenueHunt of a verified deletion request relating to quiz information it processes on our behalf.

Information stored in archived or backup systems may be deleted on a delayed schedule where applicable law expressly permits additional time for backup or archive deletion.

Withdrawal of Consent

If you withdraw consent for future collection or sharing, the withdrawal will apply prospectively to the extent required by applicable law.

Appeals

If we decline to take action on a consumer health data request and applicable law gives you a right to appeal, you may appeal our decision by emailing myrevitaplus@outlook.com with the subject line “Consumer Health Data Appeal.”

We will review the appeal and provide a written response within the period required by applicable law. If an appeal is denied, we will provide information about how to contact the appropriate state regulator or attorney general where required.

We may update this Policy when our consumer health data practices or applicable legal requirements change.

The current version will be posted on this page with an updated effective or revision date.

Where required by applicable law, we will provide additional notice and obtain affirmative consent before collecting, using, or sharing additional categories of consumer health data or using consumer health data for new purposes that were not previously disclosed.
